Are you ready to schedule your appointment?
At Premier Dental, we are happy to reserve a dental appointment especially for you. Just complete and submit the form below, and someone from our office will contact you to confirm your appointment.
We do ask if you need to change your appointment time, to please give us at least a 24-hour in advance notice. This allows your time to become available to another patient. We do, however, understand that some emergencies do come up, so please make every effort to let us know.
Delays do occasionally occur, even though we make every effort to see a patient at their appointed time. These delays usually occur as we are trying to accommodate a patient having a dental emergency—we do appreciate your understanding in these instances.
